In this video we will discuss the formally published Final Report from the ACTT-1 group on Remdesivir to treat COVID-19. This was a therapy that President Trump received while hospitalized at Walter Reed. It is a viral RNA dependent RNA polymerase inhibitor that has proven efficacious in cell culture against SARS-CoV-2 virus. We will talk about this medication and how it works. The ACTT-1 trial was a larger randomized, double blinded, placebo control trial performed at 60 sites all over the world. What they found was that Remdesivir decreased duration of symptoms and trended towards a mortality improvement. We will dive into this trial. What do these conclusions mean?   1. I discerned no bias. I have stopped giving it because there is a problem showing mortality benefit, just like the plasma trial. I feel Ivermectin would trend or better, but no studies done conclusively. I may reconsider those on oxygen alone, ( the bulk of hospital cases ), but in the era of dirt cheep steroid and MATH + therapy, that seems to be working, why add 1,600 dollars and 5 days IV to the bill? When randomized, controlled trials, of this and plasma, with thousands of patients are in, and not statistically significant, what are we debating about? These perks would NOT be extended to HCQ or Ivermectin, just sayin’!

  2. It appears that remdesivir is better than nothing in treating COVID-19. But its true benefit may be in helping to determine whether other factors, yet unknown (genetic, behavioral environmental, etc.), are influencing the progression of the disease.

  3. Was this study flawed from the start due to the fact that the median antiviral intervention started 9 days after symptoms? By symptomatic day 9, is the viral replication phase already starting to wind down?

    Any thoughts on better outcomes if the intervention started several days earlier when the virus is spending a great amount of its effort on replicating…say 5 or 6 days after symptom onset instead of 9?
